Are you now on an Assignment with CTC recruitment? Although we will have sent you all the information you require by email or post you will also find most of the essential information and documents on this web page.

PAYE:

If you are being pay rolled by CTC recruitment we will require you to complete the following documentation and return it to us to enable us to pay you, (2-5 are generic documents):

1. Supplementary Terms and Conditions for Services for Temporary Workers: these outline the rate of pay, location of work and hours of work. They are specific to your Assignment and are only available from our office,
2. CTC Contract for Services for Temporary Workers (Terms of Engagement)VO6;this is available here,
3. Confidential Payroll Information Form; this is available here,
4. Fitness to Work Questionnaire; this is available here and is a standard document which applies to all Assignments,
5. P46: this is available here,
6. Timesheet: where advised some candidates will need to submit a client authorised timesheet to CTC on a weekly basis. A generic timesheet is available here.

In line with the requirements of our clients and government regulations we are required to keep on file a copy of the following documents:

• Passport or Birth certificate,
• Driving Licence (if applicable)
• National Insurance card or P60 or P45,
• Copies of relevant qualifications,
• Names of two referees, (to include previous employer),
• LOLER Record of Thorough Examination (if applicable),

UMBRELLA COMPANIES:

If you work on an Assignment with CTC as an umbrella company you will be required to complete the following documentation:

1. Assignment Information Sheet: this will outline the rate of pay, location of work and hours of work. They are specific to your Assignment and are only available from our office,
2. CTC Terms of Engagement for Umbrella Workers: this is available here,
3. Fitness to Work Questionnaire; this is available here and is a standard document which applies to all Assignments,
4. Timesheet: where advised some candidates will need to submit a client authorised timesheet to CTC on a weekly basis. A generic timesheet is available here.

LTD COMPANY CONTRACTORS:

If you work on an Assignment with CTC as a Ltd company contractor you will be required to complete the following documentation:

1. Supplementary Conditions of Contract: this will outline the rate of pay, location of work and hours of work. They are specific to your Assignment and are only available from our office,
2. CTC Terms of Engagement for Ltd Company Contractors for Supply to Clients: these are available here,
3. Fitness to Work Questionnaire; this is available here and is a standard document which applies to all Assignments,
4. Confidential Payroll Information Form; this is available here,
5. Timesheet: where advised some candidates will need to submit a client authorised timesheet to CTC on a weekly basis. A generic timesheet is available here.
